Help with Support Plans and Self Assessments:
The Information and Advice Service staff can support you to complete your Self Assessment Questionnaire or your Support Plan.
A Self Assessment Questionnaire is a document which helps the London Borough of Sutton to understand what support you may need and estimates the risk if help is not provided.
A Support Plan is the name of the plan that you will create following your Self Assessment and when you know what budget is available to you. The plan will give you an opportunity to think about what you want to get out of your support such as maintaining your health, learning new skills, undertaking activities etc. The Information and Advice Service holds a considerable amount of information which could help with your planning. For example accessible transport companies, accessible holiday locations, local activities such as sport or education, volunteering organisations - most will have their costs attached in order to help you budget your money.
